stop dog weeing in the house?

i have had my dog for 11 years now...she is a lakeland terrier cross and female. i love her to pieces but i can not cope with her weeing in my house anymore. it is something she has always done (ive had her checked and can never find anything wrong apart from anal glands need doing every summer..i do this myself as i am i vet nurse) i have tried everything and i really am at the end of my tether now!! i have a baby on the way and its really stressing me out. she does it in the conservatory on the laminate flooring and the rug under the dining table when we are not home. sometimes she also does it in front of us around the house. she is let out several times every hour as i am a housewife, i have another dog that does not do this (i have taped them on camcorder) and im just so confused and sick of shampooing my carpets. i am considering finding her a new home which upsets me greatly..but me and my partner can no longer cope =( and i dont want it around my child. .....any idea's? please help...thanks...(i have tried all sprays, changed my carpets, cleaning products, rugs, air fresheners, left her outside, left doors open which resulted in her weeing on our sofa...and anything else you would think of doing =/ )

No stupid comments or suggestions please...this is a big deal to me as i love her loads and she has been a big part of my life for a long time...i have had expert advice and friendly but nothing is working..this is my last resort =(....
i have tried rubbing her nose in it and punishing her...but she is not bothered in the slightest...i did the ' crate method ' when she was a puppy and it worked brilliantly...she started doing this at around 3 years old and has not stopped since...it is ruining our house!...As i said, im a qualified vet nurse, so i can assure you i have done all i can think of. x
Avo- First of all i have NEVER hit my dog...and rubbing there nose in it does not mean im bashing her head on the floor!...i make her look at it and call her a naughty girl...as i said, i am a vet nurse and love animals very much...and as for your comment of how i speak to my dog and her being scared of me and also your comment of i should of asked this question a long time ago...you clearly did not read my question properly....i have stated that she was trained properly (by myself who is qualified in doing so) i also stated that she is checked by the vet and me on a regular basis...and my animals are not scared of me in the slightest! all i do to tell them off or make them stop something is clap...they are very well trained....hense me being so confused. as i stated, i have put it on here as a last resort as im about to have a family and dont want it around the house. read things properly before you answer a question. thanks!
and...she does ask to go out...i say 'wee' and she runs to the back door...if she wants the toilet she will sit by my feet and stare at me...the same as my other dog...and this is why im so confused!...ive spent £100's on tests and even more on x-rays and she is fine!...
